Vineyard Grove Speed & Agility — Liability Waiver & Release
By registering my child for the Vineyard Grove Speed & Agility program, I acknowledge and agree to the following:
Assumption of Risk. I understand that speed and agility training involves physical activities including but not limited to running, jumping, and agility drills. These activities carry inherent risks of injury including but not limited to sprains, strains, fractures, and other physical harm. I voluntarily assume all risks associated with my child's participation.
Release of Liability. I, on behalf of myself and my child, hereby release and hold harmless Coach Dan Spirgen, Vineyard Grove Speed & Agility, and their agents, volunteers, and affiliates from any and all claims, damages, losses, or liabilities arising from my child's participation in the program, except in cases of gross negligence or willful misconduct.
Medical Authorization. In the event of an emergency, I authorize program staff to seek and obtain emergency medical treatment for my child. I understand that I am responsible for any medical costs incurred.
Photo/Video Consent. I consent to photographs and videos of my child being taken during sessions for use in program materials, leaderboards, and social media.
Fitness to Participate. I confirm that my child is physically fit to participate and has no medical conditions that would prevent safe participation, or that I have disclosed such conditions to the coach.
